We understand that your treatment is extremely stressful.  We want to be able to help you on your journey.  Please note the following guidelines for your request

In order to be eligible for financial assistance from The Toledo Clinic Foundation, you must:

  • Be in active treatment for a hematology/oncology specific diagnosis or within 3 months of chemotherapy/immunotherapy

  • Complete an application form for assistance

  • Provide a bill for the requested payment. The bill must be for a non-medical expense and specify a postal address where payment can be sent (as opposed to online payment).

The Foundation can assist with the following:

  • Rent or mortgage payment (A copy of a signed lease agreement must accompany the application)

  • Utility bills including one phone (either landline or cellphone)

  • Car payment

  • Emergency car repair

  • Car insurance

  • Rental Insurance

  • Home insurance

  • Child care/Elder care (to a licensed provider with documentation)

  • Groceries

  • Transportation to and from appointments via cab service or gas station gift card (Appointment schedule must be provided)

  • Any non clinical expense (i.e. non medical)

We are not able to pay the following:

  • Credit card bills

  • Tax bills

  • Life Insurance

  • Entertainment bills (cable, DirecTV, Internet, etc.)

  • Condo or neighborhood association fees

  • Health insurance

  • Any services or reimbursement to providers, pharmacy, etc. from The Toledo Clinic

The bills you submit will be paid to the landlord or company. We cannot reimburse our patients.

All assistance is subject to a lifetime benefit limit.
